Captopril (rINN) (pronounced /ˈkæptəprɪl/) is an angiotensin-converting enzyme inhibitor (ACE inhibitor) used for the treatment of hypertension and some types of congestive heart failure. The drug works by relaxing the blood vessels, allowing them to dilate and ease blood flow. It may take up to several weeks to achieve full benefits even though captoril quickly lowers blood pressure. If you have suffered heart failure you could be given captopril as well as a diuretic as it can achieve excellent results in relaxing muscles in the walls of the blood vessel which in turn reduces the workload on the heart. The first dose would be small and taken it while lying down would be of benefit as there is a possibility that it may cause a sudden drop in blood pressure. Some people may suffer from side effects if this is so then a reduction in the amount taken may be of help.

A major risk with this medication is that blood pressure may fall to low. Because of this reason when taking the first dose of captopril you would be advised to sit or lie down. If you have unstable blood pressure this medication would be prescribed with caution.

Prolonged Use

You may experience monitoring to check on potassium levels, white blood cell count, kidney function and urine. Prolonged use can lead to changes in the blood count or kidney function. However, this would be quite rare.

Driving, Hazardous Work & Alcohol

You should avoid these activities until you know how captopril affects you, as there is a risk of fainting and dizziness. Alcohol should be avoided as there is an increased risk of adverse effects as well as lowering blood pressure.

Pregnancy and Breast-Feeding

Would not usually be prescribed as there is evidence of harm to the developing fetus. The drug passes in to the breast milk. However, at normal dosage it would be unlikely to cause unwanted adverse effects on the baby.

Surgery

Before any surgery discuss this medication with your doctor or dentist etc. as it may need to be stopped prior to an operation.

Tell Your Doctor

You should tell your doctor if you have a current health problem. For example: heart disease, kidney problems, if you are pregnant or you intend to become pregnant, or if you think you may be at risk from drug treatment for any other reason. Some types of medication can cause serious adverse effects when taken side by side so it is important to tell your doctor of any other medication you are already taking. This should also include any over-the-counter medication. For example: Painkillers, cold remedies, herbal and complimentary / alternative medication.

Over 60’s & Children

There may be a need to a reduced dosage for the over sixties. It would not be usually prescribed for infants or children. Although a reduced dosage may be necessary.

Overdose

Large overdoses may cause dizziness or fainting notify your doctor. An extra unintentional dose would be unlikely to cause problems.

Other Medication Reactions

Non-steroidal anti-inflammatory drugs (NSAID’s) May reduce the effectiveness of captopril. If taken with captopril there is a risk of kidney damage. Captopril may raise the blood levels if on Lithium. Blood pressure can be reduced further by vasodilators. (nitrates) If diuretics are taken alongside captopril it could cause a rapid fall in blood pressure. When captopril is taken with circlosporin there can be an increased risk in high potassium levels. Potassium supplements and potassium sparing diuretics increase the potassium levels when taken with captopril.

Missed A Dose

Take the missed dose as soon as you remember. However, if it is almost time for the next dose, skip the missed dose and continue the regular dosing schedule. Do not take a double dose to make up for a missed one.

Reducing or Stopping Medication

It is important that you do not stop taking the drug without consulting your doctor. as it can lead to a worsening of the condition.

Effect Of Medication

The effects of Captopril can be felt with 30 to 60 minutes. The lasting effects of this medication can be between 6 to 8 hours.

Storage and Safety

Keep medicine in its original container, in a cool dry place, away from the reach of children. Never take medication that has past the expiry date shown on the box or container. Return all un-used or old medicines to your pharmacist. Do not discard old or unwanted medication into your trash, sink or toilet.
